By joining Colorado Physician Partners, your provider is given an increased opportunity to improve care for our patients and create innovative solutions to problems. This includes efficiencies such as centralized billing. Colorado Physician Partners practices continue to be independent, which means not owned by a hospital, insurer or outside corporation. 

No. Your provider will not change.

No. Your co-pays and out-of-pockets costs will not change because your provider joined Colorado Physician Partners.

Colorado Physician Partners is contracted with most major commercial health insurance plans including Aetna, Anthem, Cigna and UnitedHealthcare, as well as an AARP® Medicare Advantage plan from UnitedHealthcare. If you wish to view or receive a copy of your medical records, Colorado Physician Partners must obtain your written permission before releasing any copies. Please contact your provider to request the form and for instructions on how to submit.

The name we bill you under your insurance is changing to Alpine Medical Group Colorado dba Colorado Physician Partners (or a modified version). This will appear on billing and credit card statements and your explanation of benefits. As part of this change, statements for visits in 2023 or prior will be green with a return address of Lakewood, CO. Future dates of service will be blue with a return address of Phoenix, AZ.
